TIPA works with community convener THINC on the advanced primary care pilot under the auspices of the Hudson Valley Initiative, a collaborative effort to find solutions to today’s health care challenges. The THINC Quality Committee obtains input from across the health care spectrum, including health plans and providers, while the Taconic IPA obtains input from its Medical Council members on program design, operations and evaluation of the pilot. Experienced TIPA staff provides the hands-on operation and oversight of the embedded case managers working within participating pilot practices. TIPA serves as an active participant on the THINC Quality Committee as it seeks to improve care coordination over the course of the project. TIPA also created and is convening a Care Coordination Working Group as part of the pilot.
Using a work group model, TIPA fosters communication, coordination and collaboration between the practice embedded case managers, practice and health plan case management leadership and community-based case management leaders such as hospital and home care providers. This forum is utilized to share best practices that are developed, address operational issues as they arise and implement enhancements to workflows across settings that result in quality improvement. The work group engages providers as they seek innovative ways to integrate effective care coordination and community-based support strategies into primary care practice workflow and ensure transitions of care strategies are in place across delivery settings to coordinate patient care.
