Quality health care should be patient-centered, coordinated, accessible, low-cost and high quality. Over the past decade, TIPA has made tremendous advances in an intentional effort to achieve this aim. Our foundation in health information technology is strong, allowing us to drive meaningful improvements in patient care. We are building an approach to better coordinated care enabled by technology that reduces fragmentation and closes gaps in care 

Transforming the health care community is a journey. TIPA brings resources and capabilities to aid practices on the implementation pathway toward advanced health care practice, embodied in the patient-centered medical home and advanced care coordination models—all in an effort to gain more value from the health care system, for individual patients and the population of the Hudson Valley. TIPA works with research partners to evaluate the lessons learned along the way, and to measure improvements in care.
