In keeping with its mission to optimize the quality and value of medical services, Taconic IPA has been a member of the Institute for Healthcare Improvement's Triple Aim project since the beginning of 2010.
The Triple Aim approach suggests that overall health care system improvement requires simultaneous change on three fronts: improve the health of the population; enhance the patient experience of care; and reduce, or at least control, the per capita cost of care.
Together with teams from across North America and Europe, TIPA is committed to achieving better health, better care, and lower costs for the people of the Hudson Valley. A system built with a foundation in robust primary care, properly designed and financed, can serve as the cornerstone of change. TIPA's work in helping a number of practices achieve NCQA Level 3 Patient-Centered Medical Home recognition, its work to improve care coordination, and its inclusion of public and private providers and payors in its quality improvement efforts make it ideally suited to participate in this nationally groundbreaking work.
TIPA participates through work groups in three areas of IHI's Triple Aim project:
Each work group participates on a monthly call and often tests ideas and improvement approaches between calls. TIPA participated in the Triple Aim face to face meeting in Denver in June 2011, presenting its work to date.
