Imtiaz A. Mallick, MD, FACP, Chairman
Imtiaz A. Mallick is a graduate of King Edward Medical College with subsequent training at New York
Medical College and Westchester County Medical Center. He has been practicing Internal Medicine
and Aviation Medicine in Fishkill, New York. He is a diplomat of the American Board of Internal
Medicine and a Fellow of the American College of Physicians.
He is the president of the Central Hudson Valley Chapter of the American College of Physicians and
the current president of Dutchess County Medical Society.
Dr. Mallick has been actively involved
in the inpatient medicine as a Chief of Medicine at Vassar Brothers Medical Center. He has
vast experience in managed care as a Medical Director for MVP Health Care since 1996 and has
chaired the Taconic IPA Credentials Committee for a number of years. His Aviation Medicine
interests have resulted in his appointment as a Senior Aviation Medical Examiner by the Federal
Aviation Administration and he is an avid aviator with Commercial Pilot, Single Engine Flight
Instructor and Multi Engine Flight Instructor ratings.
Dr. Mallick's practice is one of the
practices participating in the Patient Centered Medical Home project. He has been conducting
a month-long charity medical mission annually to the medically underserved regions of the world
for the last twenty one years.

Toni A. Saychek, MD, FACOG, Vice-Chairwoman
Dr. Toni A. Saychek received her undergraduate degree from Vassar College, followed by her MD from SUNY Stony Brook School of Medicine in 1983. She completed her internship in Obstetrics and Gynecology at New York University – Bellevue Hospital Center in 1984. Dr. Saychek was a resident in Obstetrics and Gynecology at New York University – Bellevue Hospital Center from 1984-1987.
Dr. Saychek is Board Certified in Obstetrics and Gynecology. She is affiliated with Vassar Brothers Medical Center. She was the Assistant Chairman of the Department of OB/GYN at Vassar Brothers Medical Center from 2005-2006 and the Chairman of the Department of OB/GYN at Vassar Brothers Medical Center from 2007-2008. Dr. Saychek practices in Fishkill and Poughkeepsie.

Mark A. Fish, F.S.A., M.A.A.A., Treasurer
Mr. Fish is the Executive Vice President of Network Management for MVP Health Care, Inc. He holds a Bachelor of Business Administration degree in Actuarial Science and a Bachelor of Science
degree in Mathematics from the University of Wisconsin. He is a Fellow of the Society of Actuaries, a member of the American Academy of Actuaries and a graduate of the
American Association of Health Plans’ Executive Leadership Program.

Gregg Rockower, MD., F.A.A.P., Secretary
Dr. Rockower is a board certified pediatrician at Clarkstown Pediatrics in Rockland County. An alumni of Cornell University, he received his medical degree from SUNY Health Science Center at Brooklyn, then completed his residency and chief residency at New York University’s Tisch and Bellevue Hospitals. Dr. Rockower is a Fellow of the American Academy of Pediatrics.

Fabio O. Danisi, MD
Dr. Danisi is a native of Rome, Italy where he attended medical school. He completed a
neurology residency and movement disorders fellowship at the Mount Siani Medical Center
in New York. He practices neurology in Kingston and New Paltz.
He is principal investigator for several clinical research trials and has published
entensively in the fields of Parkinson's disease and other movement disorders. He serves
as treasurer of the Ulster County Medical Society and chairs the Kingston Hospital Institute
review board.

Dr. Ennis practices internal medicine in Yorktown Heights. He received his medical degree from New York Medical College and completed his residency at Westchester County Medical Center.

Lewis Kohl, DO
Lewis Kohl, DO, of North Salem, is the Vice President of Medical Affairs at Putnam Hospital Center in Carmel, NY. As a member of Carmel hospital’s senior management team and medical administrative officer of the organization, Dr. Kohl is responsible for strategic issues, responsible for the oversight of quality and risk management, and organizes and directs all medical and clinical activities of the hospital and its services.
Dr. Kohl has over 24 years of clinical and leadership experience. Most recently, he served as Director of Clinical Informatics, Medical Director of Emergency Medical Services and Chairman of the Department of Emergency Medicine at Long Island College Hospital. His previous affiliations included serving as Chairman of the Department of Emergency Medicine and Site Director for the Emergency Medicine Residency at Brookdale University Hospital and Medical Center, working as an attending physician at Brooklyn Hospital, and working at Kings County Hospital/SUNY Health Science Center at Brooklyn as an attending physician, Assistant Residency Director and Deputy Director in the Department of Emergency Medicine and Course Director in Emergency Medicine for Senior Medical Students.
Board certified in both emergency medicine and internal medicine, Dr. Kohl received his medical degree from the New York College of Osteopathic Medicine. His postdoctoral training included a rotating internship at Coney Island Hospital and an internal medicine residency at Kings County Hospital/Downstate Medical Center where he was chief resident.

David W. Oliker
Mr. Oliker is CEO/President of MVP Health Plan. He has led MVP since the company's inception in 1982.
He is a member of the American Association of Health Plans Policy Advisory Council and a member of the
executive committee of the New York State Health Plan Association and past Chairman.

Hal Teitelbaum, MD, MBA
Hal Teitelbaum is the Managing Partner, CEO and founder of Crystal Run Healthcare, among the largest,
fastest growing and most technically advanced medical practices in New York State. Dr. Teitelbaum completed
his training in Internal Medicine at New York Hospital-Cornell Medical Center, and in Medical Oncology
and Hematology at Memorial Sloan-Kettering Cancer Center. After beginning his career in full-time academic
practice, Dr. Teitelbaum established a solo practice in Orange County, New York, in 1982. He established
Crystal Run Healthcare in 1996. Dr. Teitelbaum is a 1998 graduate of Columbia Business School, and was
recently admitted to the Columbia Law School, Class of 2012.
Dr. Teitelbaum has served on the board of Horton Hospital and of Elant. He is a recipient of the ADL
Americanism Award, the Rhulen Award of the Sullivan County Partnership for Economic Development, and
the Alliance for Balanced Growth’s ‘Golden Shovel Award’ of the Orange County Partnership. He was
recognized by the Medical Group Management Association (MGMA) as the 2006 Physician Executive of the
Year. Most recently, Dr. Teitelbaum was among the nominees for Modern Healthcare and Modern Physician
Magazines' list of the 50 Most Powerful Physician Executives in Healthcare.
