Taconic IPA (TIPA) provides resources and technical support to primary care physician practices as they seek to achieve higher levels in quality patient care delivery. Our Practice Transformation Service works with physician practices to help achieve the top recognition from the National Committee for Quality Assurance Physician Practice Connections®-Patient-Centered Medical Home™ (PPC-PCMH) program, which provides external, independent validation that a primary care practice meets the requirements of a medical home.
The medical home model has been associated with improved quality, better patient and provider satisfaction and lower costs. TIPA has a proven track record for providing the skill, experience and knowledge needed to assess practice readiness and then help navigate the NCQA recognition process. In 2010, 236 physicians working at 51 primary care practice sites in the Hudson Valley achieved Level 3 PPC-PCMH patient-centered medical home recognition with TIPA’s assistance.
TIPA offers services to assist practices with their re-recognition efforts as well as to select practices interested in achieving recognition for the first time under the 2011 NCQA Level 3 PPC-PCMH standards.
